Below are forms you can download from HBS. You can print these and post them back to us for a variety of options.

Attention Non-Residents of New Zealand - you will not be able to complete these forms and post them back to HBS.
You will need to come into one of our branches so we can sight your identification in person before opening an account.

Adjustments to existing accounts

Notification of Change Form (449kb)

Please ensure all account signatories sign for any changes as detailed at the bottom of the form.

New Direct Debit Form (541kb)

Please ensure you also supply an operating authority from your trading bank that shows proof of signatures.

Individuals/Joint/Multiple - Personal Accounts

New Personal Account Form (475kb)

You will also need to supply Identification in the form of either a) Copy of Passport or b) Copy of Drivers Licence, your IRD number and if the account is to be operated by a person under the age of 16 the form below.

Child Indemnity Form (137kb)

To be used if the account is to be operated by a person under the age of 16 as above.

Incorporated Societies/Clubs
